This brief questionnaire will evaluate whether you may be suffering from a heroin dependency. Deriving from the opium poppy, heroin is a powerful illicit opiate drug that is typically injected or snorted, but may also be smoked—any method of administration can be extremely addictive. These simple screening questions are based on criteria of the “Diagnostic and Statistical Manual of Mental Disorders” for opioid use disorder, which includes heroin use.
