The following questionnaire is based on the factors used to diagnose substance use disorders as defined by the Diagnostic and Statistical Manual of Mental Disorders, 5th edition, also known as the DSM-5. The DSM-5 criteria are outlined for use by psychiatrists, psychologists, doctors, and other medical and mental health specialists as a standard by which to diagnose various mental health disorders, including substance use disorders.

Substance use disorders are diagnosed by medical or mental health professionals using DSM-5 diagnostic criteria after thorough patient assessment. Such diagnoses are made based on an individual meeting 2 or more criteria within a 12-month period.
